This is an excellent site which is very easy to fill out and makes the whole process of future care planning easier to discuss and to have your wishes known.

I have worked with Dementia care and End of Life care for a huge part of my working life. I have seen many clients and their families unprepared and unable to fulfil their loved ones wishes just through "not knowing". Families and Care Homes and all medical disciplines do their best but if they don't know then a lot is guess work and inadvertently it may be very upsetting or even worse for the client who maybe can no longer make their wishes and likes and dislikes known. I was determined this wasn't going to happen to me....my family know me pretty well but even so I have filled in all the forms carefully and meticulously and my next of kin know where the forms are kept. This helps to give me peace of mind should anything tragic happen to me.

Two years ago, at the age of 56 and being quite fit and healthy - I dropped down dead! Fortunately, somebody was nearby to give me CPR until the ambulance arrived. We just never know what is going to happen or when. That (and watching my poor Aunt , who suffers from dementia) is why I have now used Amber Plans free service to put my wishes down in print - giving me some peace of mind

Amber care plan is a great guide to have in place detailing your end of life wishes for your loved ones to follow, should you ever be unable to communicate them. It's very easy to complete, with step by step guidance, and you're able to make changes at any point, if you wish to do so. You can appoint an appropriate person to receive a copy of the plan, who will be able to act on your behalf to ensure your wishes are respected as closely as possible. Sections include things from food, care, and medication preferences to funeral wishes and you can also add any specific details which may not have been covered.
Definitely worth doing to spare your loved ones having to make those difficult decisions on your behalf.

This plan is very simple to do, and as I am looking after my father with dementia, the importance of having a plan written down and shared with the family, is so valuable. I only wish he had been able ot do his own plan before the dementia got so advanced.
Very easy to do. The whole process was so simple. Having just been diagnosed with a chronic kidney disease, knowing that my preferences and wishes are clearly laid out to aid my family gives me some small comfort. Highly recommend. Everyone should have one.

Making a care plan is easy enough. The form is split into different sections and you don't have to do them all at once. It is also easy to change any decision/s in the future. My hope is this wil help inform my children what sort of care I do and don't want, should it be needed.

A great idea which every adult should do what ever their age. Working in health care and having had this difficult question asked in my Mum and Dads hour of need, I realise how important it is to have you wishes down in writing way before you need them. I would hope my family would do the right thing but instead of forcing them to make difficult decisions when they are already vulnerable I now have my wishes known taking their stress away a little. I hope Amber care plans add a page to address people's preferences regarding funerals, music etc but that may come in time. Please every one fill in this page, help your nearest and dearest and get your views on this difficult subject known.
