Community Mind Thrive is a nurse-led, community-based early mental health and emotional wellbeing service supporting adults aged 18+, with a particular focus on young adults. We provide accessible, preventative support at an earlier stage because we believe people should not have to reach crisis point before receiving help. Our approach is LGBTQ+ friendly, neurodivergent-friendly, culturally responsive and trauma-informed, creating a welcoming and non-judgemental environment where people from different backgrounds and life experiences can feel heard, understood and supported. We recognise that mental wellbeing is often affected by more than symptoms alone. Young adults may be navigating anxiety, low mood, stress, loneliness, relationships, university or employment pressures, housing difficulties, financial worries, identity, migration, major life changes or social isolation. Through one-to-one emotional wellbeing support, psychoeducation, peer support, mentoring, community wellbeing sessions and supported signposting, we help people understand what they are experiencing, develop coping strategies, build resilience and reconnect with their community. Community Mind Thrive also works alongside GP practices, social prescribers, NHS services, local authorities and community organisations to strengthen referral pathways and help people access the right support at the right time. Our mission is to intervene earlier, prevent crisis, reduce isolation and empower young adults to move forward with greater confidence, resilience and hope. We heal • We grow • We thrive.
