This has been a wonderful program. Frequent conversations with my doctor and nutritionist have been very helpful. The one thing that needs improvement is the app, but that’s minor.

They could not submit convincing prior authorization paperwork for my insurance to cover my Zepbound 4 months ago.

Having a team guide me through this process is what makes Form worth it! They are responsive, resourceful and available for as much (or as little) assistance as I request. Every month, my request for Rx refills has been easy and timely.

Horrible experience, I signed up and paid the fee and uploaded my drivers license and insurance and got no where. I woke up to a message saying my membership was terminated. No reason given. Never got any appointments or medications. I think it’s unnecessary to have a grown adult be forced to see a PCP just to get weight loss medication. I never go to the doctor and avoid doctors appointments because it takes most of the day sitting in a waiting room then paying someone to treat me like poop. No thanks!!! They stated that you absolutely had to be “under a doctors care” like Im an adult I don’t need a doctors approval to exist an d make decisions for what I want to do.

The medical care team keeps getting switched and there is no timely follow up

Easy straightforward process, does take a little bit of work in time to get all the paperwork ready but once you do, everything else goes quickly and smoothly and so far everyone I worked with is extremely helpful, nice polite, and informational.
