When I called Choice, I spoke to Lucas and explained to him what I needed, in a very polite manner he conducted the usual background checks to verify who I was, then he verified my needs and told me he would email these to me in about 10-minutes. I received the forms via email before the 10-minute period. Thank you, Lucas!!
