Every individual human I've worked with has been kind (if a bit rushed), but the overall experience has been a nightmare of unclear privacy, unclear expectations, unclear obligations, and needlessly frequent high touch interactions.

I find Form Health to be adding a lot of wasted time and resources (both electronic and physical -- they sent me a scale without confirming with me first for example), overbroad permissions (they wanted blanket access to my medical records), and difficulty finding providers (the first providers they offered were only available at 5AM).

They've leaked my personal health information (in ways that were quick to clean up, but still... that's careless and unacceptable) and overall given the impression of a startup who is out here faking it until they make it.

This definitely doesn't feel like a mature healthcare institution operating with my interests at heart. I would recommend to my friends, colleagues, and family to consider any route that isn't Form Health.

Edit (In response to their reply): These concerns come from dealing with several attempts at reaching out to get them addressed through the email address they provided. The review reflects my experience and opinion *after* I've tried to improve and rectify them by working with Form Health.

Each meeting is personal and meaningful to me. I enjoy the communication between the team members understanding, and listening to my situations each visit.

I will admit I was skeptical, because I was forced to join Form Health by my employer if I wanted to continue my journey on a GLP. I have been so pleasantly surprised! Form Health Chivas is amazing, she listens to my concerns and questions, and is extremely understanding. My nutritionist, Molly has been incredible. She has provided great advice and patience as I have been dealing with g with some personal things. I am looking forward to working with them in 2026 to reach my goals!

The staff at Form Health are great! I really appreciate having both a doctor/PA and a nutritionist to keep me accountable on the program. The app is extremely easy to use and the resources it provides are very helpful.

Easy and simple

The website is easy to use, the providers are responsive to questions, it’s been a good experience so far.

I tried other online diet and nutrition sites in the past. Form Health, their doctors and nutritionists are by far superior!
They actually care about you, your health and your journey to a better you.
They encourage you, listen to your concerns and guide you to make better choices in your meals without shaming you. I’ve lost almost 10 lbs so far and loving it!

Great providers and quick response time to any questions/concerns.

I wanted to share my experience with losing weight since joining Form Health. When I started the program on September 26, 2025, my weight was 241.7. Not quite three months later; December 8th, 2025, I have lost 32.9 pounds. I have not experienced any issues, problems, or concerns while taking the Mounjaro medication during my almost three-month journey. I have the support of a nutritionist whenever I have questions about meal planning, and frequent check-ins with a physician to reassure and encourage me throughout the process. I highly recommend joining Form Health if you are serious about losing weight and taking control
of your eating habits. My journey has been exciting and has given me a sense of accomplishment knowing that I am doing something good for my overall health, as I continue to work toward maintaining a healthy lifestyle.
