I love my care team! They are so supportive and helpful. I've had some health challenges during my weight loss journey and my care team has worked around those challenges with me. I feel they truly have my best interests at heart and want me to be healthy and succeed.

I love that I am getting support through my journey. My doctor, nurse practitioner and dietitian are absolutely amazing. If I have any questions or concerns they quickly respond and help me out.

No Call Backs
Unfortunately, I am unable to provide a positive review so far. As a new subscriber, I have been trying to connect with the Form Health team for over a month leaving 4 messages within the last week and a half with no return call. I am beyond frustrated but am stuck using this service for now.

A great team that goes the distance with you
I have been working with Form Health for 2+ years and would absolutely recommend them to anyone who desires to lose weight — and keep it off — in a healthy manner. This program helped me lose 30+ lbs and I am grateful for the accountability and cheerleading via convenient and regular virtual check-ins with my dietitian, PA and MD. They are superstars who have equipped me with the tools I needed to get to a healthy weight and improve my fitness level.

The app doesn’t actually help you do anything. You have to talk to the support team to do everything. Most of the doctors are rude and dismissive.
